All support coordinators are qualified, experienced, and passionate in specific areas.
As a collaborative team, we often work together to expand our options when managing complexitites in a participant’s environment. This ensures that the participant is matched with the most qualified and dedicated worker whilst always accessing the vast knowledge of all coordinators. Due to our collaboration, we have been able to develop processes that decrease time spent on a task, which decreasese the cost to participants.
Networking
We have a vast network in and around our local area.
With extensive experience breaking down barriers within the system, we can get the best outcomes for participants who have additional complexities in their environment. We have a history of transitioning long-term hospital patients into the community, setting them up in supported accommodation, assisting their amilies to support them, and establishing sutainable networks that ensure the person has ongoing access to social and economic participation.
Relationships
Our support workers are training or have trained in the field and are passionate about their work.
They work closely with clients, their therapists, informal carer and coordinators to ensure the most collaborative approach. Our focus is matching the support worker and participant to increase the chances of developing trusting and ongoing relationships that achieve outcomes.
